OVERVIEW
The Lake Worth Bullfrogs Youth Association provides sponsorship for registration fees to children, who without financial assistance would be unable to participate in our sports programs. The Lake Worth Bullfrogs Youth Association is a 501(c)(3) non-profit organization with limited funding available for sponsorship. No guarantee of assistance is implied by this application. If the number of sponsorship applications submitted an approved exceeds the amount available, the sponsorships will be awarded by a board of director vote based on need. The Lake Worth Bullfrogs Youth Association does not and shall not discriminate on the basis of race, color, religion, gender, gender expression, age, national origin (ancestry), disability, marital status, sexual orientation, or military status in any activities or operations. We are dedicated to fostering an inclusive and welcoming environment for all members of our staff, clients, volunteers, subcontractors, children, and vendors. You must apply for the scholarship by regular registration cutoff dates for your sports program. Applications close for each sport at regular registration cutoff
ELIGIBILITY
Requirements for eligibility:
- Athletes must be of eligible age to participate in a Lake Worth Bullfrogs Youth Association program.
- Parents/Guardians commit that the athlete will attend a minimum of 90% of all scheduled practices and games.
- Parents/Guardians agree to volunteer 8 hours, per scholarship recipient. Hours will support Lake Worth Bullfrogs Youth Association related activities and must be completed no later than 1 month after the sport in which the scholarship was granted has completed.
- The maximum amount awarded per recipient family is $599.00 per calendar year. Per IRS guidelines, the Lake Worth Bullfrogs Youth Association is required to issue a 1099 form to any individual that has received $600.00 or more in funds or compensation in a calendar year.
QUALIFICATIONS
Sponsorship consideration will be given to families that meet the following criteria:
- Provide a copy of your IRS form 1040 from the recent tax year.
- Receives assistance and can provide written documentation from programs such as: Food Stamps, Medicaid, SSI, Foster Care, WIC, etc.
- Provide recommendation by school representative, social worker, church pastor, youth community center workers, or other social service representative.
- Provide a written statement of immediate financial hardship explaining the current situation.
- Lake Worth Bullfrogs Youth Association recognizes that a family may not be receiving formal assistance from the programs mentioned above, yet financial assistance may still be needed to participate in a program. In these instances, the Lake Worth Bullfrogs Youth Association board of directors will consider the financial hardship statement to determine sponsorship eligibility. Please provide any supporting documentation that may support the facts in your financial hardship statement.
- Incomplete applications will automatically be denied.
